Sample Jersey or Uniform Order Form

This form lets you get orders from your members for something like jerseys/uniforms. It collects name, phone, address and their preferences for jersey size, a second jersey size, and bottoms/pant size. We have added a short description where you could link to a size guide for each piece and also shown a way to do a more complex check box grid that allows for orders of 1-3 jerseys of different sizes.
